Monday Night Volleyball Club
In consideration of my being permitted to participate in the above league/clinic of GLV, Inc., an Illinois Corporation of West Chicago, Illinois (the "Company") and other good and valuable consideration, I hereby agree as follows:
To the fullest extent permitted by law, I hereby consent and agree to assume all responsibility for any and all risks of damage or injury to my person or my property that may occur in, on or about the facilities used in connection with this league/clinic and that I use such facilities, equipment and instruction at my own risk in this league/clinic. To the fullest extent permitted by law, I hereby fully and forever release and discharge the Company and all associated entities, their owners, employees and agents from any and all liability, claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated or unanticipated, which may accrue to me, for any damage or injury which I may receive either in, on or about the facilities or while utilizing the facilities, equipment or instruction of the Company.
I do hereby acknowledge that I am the sole judge of
my own physical abilities and condition to engage in the league/clinic
for which this Release is given. No representation or warranty of
the league/clinic's suitability for me has been made by the Company.
